The Hospice & Palliative Care Team

Our interdisciplinary care team works together to ensure every patient receives individualized, compassionate support. This team may include:

  • Physicians & Nurse Practitioners – Overseeing medical care, treatment, and symptom management.

  • Skilled Nurses – Providing hands-on medical care and regular health monitoring.

  • Certified Home Health Aides – Assisting with daily personal care needs to ensure dignity and comfort.

  • Social Workers – Offering counseling, community resource connections, and support for practical and financial concerns.

  • Chaplains & Spiritual Counselors – Providing spiritual and emotional guidance according to patient and family wishes.

  • Volunteers – Offering companionship, respite, and support through meaningful presence and assistance.

Hospice and palliative care share the same heart of compassion, but they serve patients at different points in their healthcare journey.

Hospice care focuses on comfort and quality of life for patients with a life-limiting illness. Our goal is to ensure that your loved one’s end of life journey is filled with dignity, compassion, and peace, right at home, surrounded by family and love.

Palliative care is designed for patients at any stage of serious illness who need relief from symptoms, stress, or discomfort. Unlike hospice, it can be provided alongside curative treatment.

Hospice Care vs. Palliative Care
