Skilled Services

Skilled Nursing Care

Our licensed nurses provide expert medical care tailored to each patient’s condition and recovery goals. Our skilled nursing team ensures that patients receive compassionate, professional care that promotes healing and peace of mind. 


Some services include but are not limited to:

  • Advanced wound care and dressing changes

  • Post-surgical monitoring and recovery support

  • Chronic disease management (such as diabetes, COPD, heart disease)

  • Medication management, administration, and teaching

  • Patient and family education for safe self-care at home

  • Vital signs monitoring and health assessments

Physical Therapy

Our physical therapists help patients restore strength, mobility, and confidence. The goal is to help patients move with greater independence and return to the activities and lifestyles they enjoy.


Some services include but are not limited to:

  • Personalized exercise and rehabilitation programs

  • Balance and fall prevention training

  • Pain management and mobility improvement

  • Gait training, transfers, and safe use of assistive devices

  • Post-stroke and post-surgical recovery programs

Occupational Therapy

Our occupational therapists support patients in maintaining independence and daily functioning. This therapy helps patients regain independence and abilities to perform meaningful tasks to promote comfort, function, and quality of life in everyday life.


Some services include but are not limited to:

  • Training in activities of daily living (ADLs), such as dressing, bathing, and meal preparation

  • Energy conservation and adaptive techniques for chronic illness

  • Cognitive and memory exercises for brain health

  • Home safety assessments and modifications to reduce fall risks

  • Use of adaptive equipment to enhance independence

Speech & Language Pathology

Our speech therapists provide care for patients experiencing difficulties with speech, communication, and swallowing. Speech therapy assists patients with challenges in communication, cognition, or swallowing, supports memory, attention, and problem-solving, and  restores confidence in communication and promotes safe and confident enjoyment of meals.


Some services include but are not limited to:

  • Treatment for speech, voice, and language disorders

  • Swallowing therapy to support safe eating and drinking

  • Cognitive-communication therapy for memory, attention, and problem-solving

  • Post-stroke speech and language rehabilitation

  • Support for patients with progressive neurological conditions

Home Health Aide Services

Our home health aides assist with personal care needs, ensuring comfort and dignity in daily life. Our aides bring kindness, respect, and compassion to every visit, helping patients feel comfortable and cared for in their own home.  Their presence and assistance with personal care needs ensures patients feel safe, cared for, and respected while maintaining independence at home. 


Some services include but are not limited to:

  • Assistance with bathing, grooming, dressing, and mobility

  • Help with meals, light housekeeping, and medication reminders

  • Companionship and emotional support

  • Assistance with transfers and safe ambulation

  • Support with toileting and other activities of daily living (ADLs)


Medical Social Services

Our medical social workers provide essential emotional and social support for patients and families. Medical social services extends care beyond the clinical needs of patients by supporting the emotional, social, and practical challenges of illness and recovery ensuring patients and families are supported beyond medical care, helping them navigate challenges with confidence and hope.  Their guidance ensures that both patients and families feel supported through every stage of the healthcare journey.

  • Counseling to cope with illness, life changes, and stress

  • Linking patients and families with community resources and benefits

  • Guidance on financial and care planning needs

  • Support for end-of-life care planning when needed

  • Advocacy for patient rights and family needs
