Deandra Phagan, RN

I currently serve as Director of Nursing for La Vita Bella, where I lead with compassion, integrity, and a deep commitment to both staff development and patient care. My journey in home health began over a decade ago as an RN Case Manager, providing skilled care and education to patients in the comfort of their homes. Being part of their healing process and witnessing their success has always been the most rewarding aspect of my profession.

As my experience grew, so did my passion for mentorship. I became a nurse preceptor, offering training, education, and support to new nurses entering the home health field. About three years ago, I transitioned into the RN Clinical Manager role, where I focused on cultivating staff growth, reinforcing clinical standards, and fostering a culture of continuous learning and excellence.

Now, in my role as Director of Nursing, my mission is to lead with empathy, mentor with intention, and continue to provide a supportive and caring environment where trust, honesty, accountability, and compassion are not just values, they’re the standard. I believe that great care starts with a great team, and I’m committed to helping both patients and staff thrive. I want every clinician to feel valued and empowered, every patient to feel seen and heard, and every success, whether clinical or personal, to be shared and celebrated. That’s the heart of my approach, and the reason I continue to lead with purpose.
